Waialua Bullpups Pop Warner announces its 2008 registration dates at Waialua District Park on Tuesday April 8^th, Wednesday April 9^th and Thursday April 10^th from 6:30 pm to 8:30 pm. There are limited spots per division and will be filled on a first come first serve basis. Football & Cheer; Jr. Midgets, Peewee, Jr. Peewee and Mitey-Mites. Boys & Girls ages 7-15. To help us make registration more efficient, you may download all necessary forms below, print and fill them out completely at home prior to the day you plan to register. We are also requesting that you bring your child with you as well so we may get his measurements. Downloadable 2008 forms posted below!

1. Mandatory on sign up day:

   1. Original & copy of child’s birth certificate

   2. $225 per child football, $250 per child cheer*

   3. Copy of child’s medical insurance card.

**All 2008 Waialua Pop Warner Association & Oahu Pop Warner registration forms that you may download/print from waialuabullpups.org are not to be edited in anyway. In case we do find any form to be compromised from it's original state, you may risk forfeiting your child's eligibility to participate and registration fee altogether. Print out need not be in color, black & white will be accepted as well as we realize not everyone has a color printer at home. You will need Adobe Acrobat reader to see/open each downloaded document, if you don't have it installed on your computer visit adobe.com for a free copy. Another option is you may ask your friend to print out for you. If you choose to use a PAYMENT PLAN from the FINANCIAL OPTION AGREEMENT above in section II "D" or "E", an additional form from section III- "B" (2008 FINANCIAL PAYMENT PLAN CONTRACT for CHEER) or "C" (2008 FINANCIAL PAYMENT PLAN CONTRACT for FOOTBALL) below will also have to be downloaded, printed and filled out as well.
